We determine participant eligibility for the (LQRP):
Eligibility Criteria
In order to assure the greatest likelihood of complete participant success, a stringent set of criteria have been established for the LQRP.
Inclusion Criteria
| • Mild to moderate head injury |
• Supportive family members |
| • Medical stability |
• Maximum medical improvement |
| Persistent resulting injury sequelae |
• Full commitment to treatment program |
| • Non-current seizure activity |
• Available family members
(encouraged to participate) |
| • Motivated clients |

Note: Persons with severe injuries are considered on a case-by-case basis, medical improvement is determined by traditional standards, and family support participants are appropriately instructed in follow-up procedures.
Exclusion Criteria:
- Premorbid unresolved serious psychiatric illness
- Failure to comply with a prior rehabilitation program
- Active alcohol or other substance abuse
Financial Criteria
Program costs
The cost of the intake assessment process is $1,200.00. This charge includes four to six hours on-site for clinical interviews, QEEG, testing, test materials, and three to four hours for data analysis and report generation. If the client is accepted and enrolls in the LQRP, the assessment fee is included in the total program cost indicated below.The cost for the 75-hour LQRP program is $12,150.00. As indicated above, this cost includes the initial assessment, all related testing and training supplies, and educational materials. Participants are responsible for their own meals, travel, and lodging costs.
Payment Schedule
- The initial fee of 1,200.00 is due on the day of the assessment.
- A deposit of $5,000.00 is required for the LQRP, if the assessment indicates that participation in the program is appropriate.
- The balance of $5,950.00 is due on the first day of the LQRP.
- Other financial arrangements may be negotiated in advance.
